Purpose of this Informed Consent

This Informed Consent Form is intended to ensure that you understand the purpose, process, and implications of your referral to a third-party service provider through Aspire2, a division of Switch Education recruitment. By ticking the consent box below, you acknowledge that you have been informed of your rights, the nature of the referral, and any potential risks or benefits associated with the referral process.

  1. Consent to Referral

By ticking the consent box, you agree to Aspire2 referring you to a third-party service provider for the purpose of accessing services that may include, but are not limited to, career advisory, professional development, psychometric professional development services, leadership coaching, and mentoring services. The nature of the services provided by the third-party will be communicated to you prior to the referral.

  1. Scope of Information Shared

Aspire2 will primarily share your contact information and relevant professional experience details with the third-party service provider. This information is necessary to facilitate further contact with you and to tailor the services to your specific needs. The specific details to be shared will be communicated to you before the referral is made.

  1. Confidentiality and Data Protection

Aspire2 is committed to protecting your personal information in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs). By ticking the consent box, you agree that Aspire2 may share your personal and professional information with the third-party service provider as necessary to facilitate the referral and service provision.

  1. Third-Party Responsibilities

The third-party service provider is an independent entity, and Aspire2 does not have control over their operations, practices, or policies. Any contractual relationship you enter into with the third-party is separate from your relationship with Aspire2. You are encouraged to thoroughly review all terms and conditions provided by the third-party before agreeing to their services.

  1. Acknowledgement of Risks and Benefits

By ticking the consent box, you acknowledge that there may be risks associated with engaging with a third-party service provider. Aspire2 makes no guarantees regarding the outcomes of the services provided by the third-party. Any concerns or disputes arising from your engagement with the third-party should be addressed directly with them.

  1. Withdrawal of Consent

You have the right to withdraw your consent to this referral at any time by providing written notice to Aspire2. Upon receiving such notice, Aspire2 will cease sharing your information with the third-party and inform them of your withdrawal. However, any information already shared prior to the withdrawal of consent will not be retractable.
